Apply for Christmas Assistance on the following dates:

October 3rd–7th, 2022 | 8:30am–11:30am

**Children are not allowed during Registration**
**Only one family member will be allowed per application to follow COVID-19 safety measures**

All applicants must bring the following Documentation:

Applicants must provide all applicable documentation or will have to come back -no exceptions.

Identification

  • Picture ID for Applicant
  • Verification of Address
  • Proof of Guardianship or Custody
  • Birth Certificate or Current Medicaid Card (Age 12 & Under)

Address Verification

Applications are open to Parents/Legal Guardians of Children 12 and Under Living in the Following Zipcodes:

27260, 27262, 27263, 27265, 27282, 27350, 27370

Clothing & Shoe Sizes

Applicant should be prepared to provide clothing and shoe sizes for all children ages 12 or younger. You may also suggest one item needed* by each child and a wish* for each child valued at $50 or less.
