This Online Incident Report form below was created for The Salvation Army USA Southern Territory's Safe From Harm program.

The form should be used to report all major incidents that relate to Salvation Army personnel and/or occur in Salvation Army units throughout the Southern Territory. It should also be used to report if a child or vulnerable adult participant discloses abuse, or if abuse is suspected, outside of The Salvation Army. 

We have provided a list at the bottom of this page of possible incidents that should be reported using the form. 

If you are uncertain as to whether you should report an incident or not, go ahead and complete the form anyway. 

When in doubt, fill it out! 

As a reminder, if abuse is suspected, reported, witnessed, or disclosed, it must also be reported to state child protective services. All Salvation Army personnel in the Southern Territory are considered mandated reporters by law. Non-Medical Incidents

Abuse (occurring at current location, or disclosure from another location) *It's important to note that this can be ANY type of abuse - physical, sexual, spiritual, etc. 

Blatant Neglect (personnel leaving persons unsupervised, not disclosing abuse/incidents, and/or putting people in obvious danger through their actions)

Bullying 

Drug Possession (any person on Salvation Army property)

Firearm Possession (any person on Salvation Army property)

Forced Sexual Act/Behavior

Inappropriate Use of Potential Weapon (e.g., Firearm, Bow and Arrow, baseball bat, etc. - this applies to any person on Salvation Army property)

Major Fighting (two or more people, including contact that results in an injury listed below)

Major Mental Health Issue (erratic or strange behavior that endangers others’ safety, like behavior that is unpredictable, and the person is unable to control)

Missing Person from Salvation Army care

Safe From Harm Policy Violation (inappropriate communication, improper use of social media, violation of two-adult guideline, transportation violation, etc.) 

Specialized Activity Negligence (rope/challenge courses, swimming pool, fire pit, sports areas/ranges, etc.)

Threat of Major Harm to Others (major injury, death, etc.)

Threat of Self Harm/Suicide

Victim of Human Trafficking (suspected or disclosed)


Medical Incidents 

(Primarily caused by situations out of the victim's control, including personnel negligence, abuse, physical contact, lack of supervision, etc. Also includes major accidental injuries/illnesses.)

Amputation (of body part)

Animal or Human Bites

Any Injury or Illness that results in the person/victim being admitted to the hospital

Back/Neck Injury that requires immediate professional care (EMS, hospital visit/stay)

Burns (requiring medical attention)

Death

Deep Wounds (requiring professional medical attention)

Diabetic Emergency

Fractures/Sprains

Head Injury (resulting in concussion or suspected concussion)

Heat Exhaustion/Heat Stroke (caused by dehydration due to neglect - no water, forced to continue an activity while exhibiting signs of dehydration/illness)

Near Drowning

Seizures

Severe Allergic Reaction (including hives, change in skin color, rash, gastro-intestinal symptoms, chest tightness, or any compromising respiratory status)

Significant Eye Injury (leading to temporary or permanent loss of vision)

Tooth Injury (e.g., chipping, loss of tooth, etc.)
